The registered healthcare professionals at Wellness Massage Therapy are committed to maintaining the highest standards of confidentiality, privacy and professional conduct. We collect, use, store, and disclose personal health information in accordance with Ontario's Personal Health Information Protection Act (PHIPA) and the requirements of our respective regulatory Colleges. 

• Personal and contact information: name, date of birth, address, email, phone number, and sometimes credit card number (protected in our secure server)
• Relevant Health history, consent forms and treatment notes
• Insurance and billing details needed to process your claims
• Appointment information: booking history, appointment preferences, and relevant communications regarding your care or updates to our clinic and availability

• Assess your health needs, develop treatment plans, and provide appropriate care
• Schedule and manage appointments, including confirmations and reminders
• Maintain accurate and up to date clinical records
• Process payments and submit insurance claims with your authorization
• Communicate with you regarding your care
• Meet our legal, professional, and regulatory obligations

Access to your personal health information is limited to the healthcare practitioners directly involved in your care and of the administration of your appointments. We use secure booking and electronic health records systems to protect your information and maintain the confidentiality of your records. We may share relevant health information with other healthcare professionals involved in your circle of care (Doctor, physiotherapist, chiropractor, etc) to support coordinated and effective treatment. 

If you request direct billing to your insurance provider, certain information may be shared as necessary to process your claim. Your consent for insurance submissions is obtained as part of our intake process.

Your clinical records are securely maintained and retained in accordance with the requirements of the applicable regulatory Colleges and Ontario privacy legislation. You have the right to access your records at any time, subject to applicable legal requirements. 

Once the record retention period has passed, records are securely destroyed in a manner that protects your privacy and confidentiality.
